Abstract: At approximately 10:45 a.m. on June 4, 2008, Employee #1 was working on a single family home under construction. Employee #1 was instructed by his employer to go on top of the roof and clean it using a blower. As Employee #1 was cleaning the roof, he fell through the unguarded 21-ft long by 4-ft wide skylight opening. Employee #1 fell 14 ft, 7 in. to the ground. Employee #1 was hospitalized from June 4, 2008 until June 18, 2008, when he died.
