Abstract: On August 28, 2003, Employee #1 was working on a concrete drainage catch basin using a concrete gas cutting saw to cut a notch in the concrete wall. The concrete wall was not supported to prevent the saw blade from pinching or kicking back. During cutting, the saw blade became pinched within the concrete block. He either attempted to free the blade by pulling up on the saw, or the saw kicked back, causing the saw blade to become free. The ground he was standing on had a slight slope and when the saw came free, it caused him to fall backwards. As he fell backwards, the saw struck the left side of his neck and upper shoulder areas. He received massive lacerations and blood loss and died.
