Abstract: At approximately 11:00 a.m. on November 14, 2003, Employee #1, a machine operator, was operating a Peerless horizontal band saw cutting aluminum from a 2-in.-by-14-in size to a 2-in.-by-7-in. size. The saw's coolant system was inoperable. To compensate, Employee #1 was holding a one-pound wax stick in his left hand and applying it to the moving saw blade. The wax stick became stuck on the blade causing Employee #1's hand to slip off and make contact the teeth of the saw blade. Employee #1 suffered the amputation of his pinky and ring fingers and was hospitalized.
