Abstract: On February 27, 2009, Employee #1 was working in the production department of a company specializing in industrial laundry services. As Employee #1 placed a pair of hospital scrubs onto the conveyor of a Jensen Superstar flat-work Iron, Model Number 800, the wrist band of the scrubs got caught on Employee #1's right hand. The scrubs then, in turn, became entangled around shaft "A" of the iron, pulling Employee #1 into the machine and amputating the lower portion of his right arm. Employee #1 was hospitalized, where he was treated for his injury. Shaft "A" measured 1.5 inches in diameter and powered the drive roller for the conveyor, while shaft "B," also measuring 1.5 inches in diameter, connected to the drive roller and on powered the conveyors pillar block bearing. Shaft "A" and shaft "B" were only partially guarded.
