Abstract: On May 21, 2002, Employee #1 was delivering kitchen appliances and moved an appliance to the second floor landing. The landing was 45-inches wide and 8-feet long on the back side of the landing. There was a 25-inch wide, 8-foot long hole with a drop of 10-feet to a lower level. Employee #1 stepped into the hole, falling to the lower level. The guardrail had been removed prior to the appliance delivery. Employee #1 was not hospitalized.
