Abstract: At approximately 7:00 p.m. on August 28, 2004, an employee rolled a 4,450 pound I-beam on skids. He worked at Steelfab Inc., in Charlotte, N.C. The beam was standing on its side and the employee was going to weld the top wing plate to the beam. Before the employee could weld the top wing plate, he attempted to clear the chain and hook in order for the weld not to damage the chain. When the employee was raising the chain and hook, he was standing in front of the I-beam. The chain and hook did not clear the beam and the hook caught it. When the hook caught the beam, it fell off of the skid onto the employee. The beam was about 1 to 2 inches from the edge of the skid. The employee was standing on the floor at the time of the accident.
