Abstract: On March 27, 2006, Employee #1, a maintenance mechanic, was repairing an exhaust system on a mixer that had been struck by a forklift. In order to move the exhaust unit back into place, he stood on a ladder and loosened the bolts located underneath the plenum. Employee #1 balanced himself by holding the side of the connection between the plenum and the exhaust hood unit with his left hand, and loosened the bolts with his right hand. When the plenum unexpectedly shifted, the exhaust hood fell and struck Employee #1. The tip of his right middle finger was amputated.
