Abstract: At approximately 9:14 p.m. on July 27, 2005, Employee #1, machinist was cleaning the masa-feeder machine to end the day. Employee #1's main duty was operating tortilla-making machines and then clean the masa (corn dough) feeder machine at the end of his shift. To do so, he had to first get all masa out of the feeder machine. He removed the guard from masa-feeder machine, by which the auger or screw in the machine was exposed. Then he scraped the auger surface with his bare hands to remove remaining masa that was on the auger while it was operating. During this process, Employee #1's finger was entrapped between the housing and the edge of the auger. He amputated his right index finger. The employer did not have lockout/tagout procedures and did not instruct employees to lockout the machines while cleaning or repairing the machines. He was hospitalized. The employer manufactures corn tortillas.
