Abstract: On December 14, 2004, the Irwindale Police Department reported an industrial injury by mailing a copy of their incident report dated December 10, 2004. At approximately 1:09 p.m. on December 10, 2004, Employee #1, a machine operator at a brewery, was working on a machine called a depalletizer. This machine places a "slip sheet" between layers or stacks of beer cans and then stacks these cans on a pallet. When an extra sheet of paper came down, covering the sensor of the C-2 depalletizer machine, the employee reached into the machine to clear the jam, without turning off the machine. His right arm was caught in this machine above the elbow. He was taken to a medical center for treatment where he was diagnosed with contusion to his arm and nerve damage.
