Abstract: Between 5:30 and 6:00 a.m., on October 29, 2004, Employee #1, a table saw operator, amputated his left middle finger while cutting a board. At the time of the accident, Employee #1 was cutting a board to 30 inches in length out of a board that was approximately 30.25 inches to 30.5 inches in length. The board was placed on an Altendorf table saw; model number F45, with 30 inches of its length on the right -side of the blade, and 0.25 inch to 0.5 inch, which was being cut off, on the left side of the blade. To secure the board, Employee #1 held the board with both hands; his left hand on the board on the left side of the blade and the right hand on the right side of the blade. The bottom of the guard that was to cover the saw blade was broken, which exposed the blade of the table saw. Employee #1 was wearing a glove on his left hand and the exposed saw blade caught the glove, which resulted in the amputation of his finger. He was treated for the injury and released.
