Abstract: On July 20, 2004, Employee #1 was adjusting a pile of raw stock paper that was being fed into a labeling machine. He was standing on a rolling stock table, under an adjusting platform, approximately 16 inches above the floor while making the adjustments. The table was free rolling and the employee lost his balance when the table shifted. While trying to regain his balance, he placed his hand on an overhead ledge where an unguarded chain and sprocket drive was located. Two fingers on his right hand were amputated and he was hospitalized. The pinch point was located inside the frame of the machine but the frame was open along the bottom.
