Abstract: At 9:00 a.m. on January 29, 2004, Employee #1, a dairy product packager, was using a Raymond side-load forklift, Model Number 537-OSR30T, to lower some butters from the upper racks in aisles #7 and #8 to be moved to aisles #1 and #2. The butters were stacked on two pallets with one pallet consisting of two layers of butter and the other had one layer of butter. The pallet containing one layer of butter was placed on top of the other pallet with two layers. As he was transporting the two pallets, to aisles #1 and #2, the top pallet moved a few inches off to the side from where it was originally stacked. The forklift continued to move forward and the top pallet hit the neighboring metal rack. This caused the pallet to bounce toward Employee #1, striking his leg. Employee #1 was hospitalized and treated for multiple fractures to his right lower leg.
