Abstract: At approximately 11:15 a.m. on March 5, 2004, an employee was supervising the mixing, dividing and bagging operations. She saw flour-collecting pans were full and needed to be emptied. Flour-collecting pans were placed underneath the conveyor belts directly below their head pulleys to catch excess flour that was falling from the conveyor belts. As she was trying to grab the pan, her hand accidentally went into the in-running side of the pulleys, which was not guarded. The employee's left mid arm received multiple compound fractures. The conveyor belt was custom made and its manufacturer's name was not known at this time. Sew-Eurodrive, Inc. manufactured the motor to the pulleys, and it had 0.5 horsepower.
