Abstract: At approximately 11:30 a.m. on July 7, 2003, Employee #1, a maintenance technician for Wynn Oil Company, was covering a 4-foot by 8-foot skylight with a tarp to reduce the heat in the kit and package line of the production department. He either lost his footing or leaned into the plastic skylight and fell approximately 22 feet to the floor. He sustained broken wrists and was transported to Long Beach Trauma Center, where he was hospitalized. The skylight did not have a safety screen, temporary cover, or railing, and the employee was not using fall protection at the time of the accident.
