At approximately 9:45 a.m. on November 9, 2002, Employee #1, a laminator, was using a cutter (Make: Harris-Seybold Division; Model: Citation CLB-5; Serial Number: 6218-1) to cut a 24-inch by 16-inch plastic sheet. This particular cutter has a two-hand control bar to lower the clamp, which requires both hands for operation, a clamp to hold the material, and a blade that does the cutting. Further, the machine has a bar pedal that can be used as a secondary control to slowly bring the clamp down to vary placement on the paper or plastic being cut. This control does not operate the blade. Employee #1 pressed the bar pedal to lower the clamp. For some reason, the tip of his left index finger was severed. The leading indicator was determined to be the lack of a guard on the cutters clamp, which has a pinch point between the clamp and the table. Employee #1 was not hospitalized for his injury.
