Abstract: At about 3:45 a.m. on November 21, 2002, Employee #1 was cleaning the conveyor belt of the accu-cool rotor flow cooling conveyor, Model 1006-8-2, Serial Number 1015, which was located on Line 1. The conveyor belt was not very dirty, so he used a damp towel to clean the teflon-like surface. When Employee #1 finished cleaning the conveyor belt, his coworkers used a pressure hose to do some unrelated cleaning and got water on it. That caused a candy-like material to be transmitted from the rollers to the clean conveyor belt. Employee #1 grabbed a clean towel and began cleaning the conveyor belt while it was running. During this time, the sleeve of his coverall got caught in the upper roller and pulled his right arm between the upper roller and the conveyor belt. A coworker heard Employee #1's screams and came to his aid. The coworker immediately pushed the stop button to stop the conveyor. Employee #1 had his arm all the way to the shoulder wedged between the upper roller and the conveyor belt. The coworker loosened the tension on the conveyor belt and removed Employee #1's injured arm. Employee #1 was transported to the Foothill Presbyterian Hospital where he remained for six days while being treated for five fractures of his right arm.
