Abstract: On August 27, 2002, Employee #1, a temporary worker with 15 days on the job, was troubleshooting a 5.5-ft-tall Ball and Jewell portable grinder (Serial Number 50966 7-M), that sat on a 2-ft-square base. Apparently, the grinder had stopped working and he had loosened two retaining bolts and removed the feed chute, exposing the rotating drum that held the blades. Employee #1 put both hands on the outside of the frame of the grinder and was looking inside when the grinder suddenly actuated, partially severing his left hand. He later stated that he was aware of the employer's safety rule that grinders were to be unplugged before any work was done on them. A supervisor noted that Employee #1 had earlier been verbally reprimanded for attempting to free a jammed grinder without unplugging it. Employee #1 stated that he had been aware of and trained in the employer's de-energization procedures, and that he chose not to de-energize the machine.
