Abstract: At about 6:40 a.m. on July 17, 2002, Employee #1 was a chemical operator at the Electronic Products Department of the company. He was operating a pump that was transferring a decant solution from a tank via ultra filtration system to a holding tank prior to disposal to sewer. At the time of the accident, Employee #1 was checking the pressure gauge of the ultra filtration system to determine if he needed to change the filter bag of the system. As he was standing in front of the system he heard a whistling sound, the pump broke down and the hot water solution (ph 9) containing traces of ammonium hydroxide, starch, nitrates and hydrazine hydrate squirted out of the pump unit onto his arms, chest and abdomen resulting in a second degree burn. Employee #1 was hospitalized for his injuries.
