Abstract: At approximately 5:00 p.m. on July 19, 2001, Employee #1 was responding to a complaint by a coworker that the boxes were coming out of the packing machine dirty. When the boxes are coming out dirty it usually means that the conveyor belts have debris or glue residue on them. Employee #1 was using a hand held damp rag to clean the conveyor belts when the rag and his right index finger got caught between the conveyor belt and a partially guarded pulley. Employee #1 suffered the amputation of the right index finger to the first joint. Employee #1 was treated for his injury but was not hospitalized.
