Abstract: At 11:15 p.m. on March 20, 2001, Employee #1, working for Creftcon Industries Inc., was injured while driving a fork lift inside the die casting area. The fork lift which he was driving was a different one from other regular fork lifts. This is a type of fork lift where operator stands instead of sitting during driving. Employee #1 stated during the interview that he was driving Fork Lift Number 01, going in reverse, to the storage area inside the building when the steering wheel got stuck and made him loose balance, causing him to fall from the fork lift, about 12 inches, and he got pinned against the metal post of a storage rack. According to his employer; Employee #1 was driving the fork lift down the center aisle going backwards, jumped off the moving fork lift, and got pinned against a storage rack by the same fork lift. Employee #1 stated that the brakes on the fork lift were functioning alright at that time and he had physically inspected the fork lift on that day prior to using it. Employee #1 fractured three ribs and suffered internal injuries after he got pinned between a storage rack and the fork lift. He was hospitalized at a local hospital for five days and released afterwards to recuperate at home
