Abstract: At approximately 8:30 a.m. on April 11, 2001, Employee #1 was cutting a piece wood with a table saw. He was trying to reduce a piece of wood with a dimension of 95 inches in length, 45 inches in width, and 0.75 inches in depth to 75 inches in length, 20 inches in width, and 0.75 inches in depth. The accident occurred when he was cutting the width. He had just finished cutting the wood lengthwise without the spreader. According to Employee #1, his hand slipped due to the WD-40 residue he had on his hand from spraying the saw earlier that morning. Employee #1 stated that he was not trained to use the guard nor had he seen the hood guard, anti-kick back, or spreader before. He sustained complex lacerations to his index, middle, and fourth fingers. Employee #1 was transported to Citrus Valley Medical Center where he was hospitalized for two days.
