Abstract: At approximately 4:45 a.m. on September 5, 2000, Employee #1 started Rewind Machine Number 1 and was checking the glue level when he heard a noise. Employee #1 determined that paper had wrapped around the belt and pulley drive of the transport conveyor. Employee #1 started removing the paper from the jammed belt and pulley drive. The belt and pulley began turning and caught the tip of Employee #1's left index finger. Employee #1 pulled his hand back and as he did the belt and pulley drive caught the tips of fingers Number 2, 3, and 4. Employee #1 did not stop Rewind Machine Number 1, did not deenergize the power source, and did not mechanically block or lockout the moveable parts to prevent inadvertent movement prior to attempting to un-jam the machine. The belt and pulley were not guarded. The belt is 1.25-in. wide and the pulleys are 14 in. center-to-center. The belt is similar to a timing belt. There are horizontal ridges across the belt and pulley. The supervisor did not see the accident but was in the area and when he heard Employee #1 yell he ran over to assist Employee #1. Employee #1 had the tips of four fingers on his left hand amputated. All four fingers amputated involved bone loss. Employee #1 was treated for his injuries but was not hospitalized.
