Abstract: At approximately 12:10 p.m. on February 9, 2000, Employee #1 was running Nile Spice Line Number 2 and noticed that the cup transfer conveyor was not working properly. He called the mechanic to repair the conveyor. After repairing the conveyor the mechanic asked Employee #1 to start the conveyor so he can check for the proper function. At this time the guard was removed from the conveyor belt for repair. Employee #1 pushed the start button with his right hand and his left hand was resting on the frame of the conveyor. As the conveyor moved, Employee #1 left ring finger was caught between the frame and a metal bar on the conveyor belt amputating it. Employee #1 was hospitalized.
