Abstract: At approximately 1:15 p.m. on August 03, 1999, Employee #1 was working on a balcony (second story, 13 ft above ground level). Employee #1 was removing the old top board (4in. by 6in. by 8-ft) of the railing for the balcony to replace it with new one. Employee #1 had used an extension ladder to climb to the balcony. After Employee #1 removed the old railings, he threw the railing board to the ground. Employee #1 was not wearing any personal fall protection and had removed the railings that could have prevented him from falling. Employee #1's right foot was on the edge of the balcony floor when his foot slipped and he lost his balance. Employee #1 fell 13 ft to the ground. Employee #1 was hospitalized for a fracture on his left elbow, left wrist, and right foot.
