Abstract: On September 2, 1998, Employee #1 was operating a Uniloy, Johnson Control, blow molding machine, identified as number four, in the production department. The right hand of Employee #1 became caught between a gap in the platen and the rotac adjusting bolt block as he tried to tighten the tie bar adjusting nut. This action was being performed while the machine was running. The third digit on his right hand was amputated.
