Abstract: At noon on August 17, 1998, Employee #1 was working on the plumber's tape machine. The supervisor had asked Employee #1 to finish a seventy box order of plumber's tape. As Employee #1 was operating the machine, he was putting stickers on the rows of plumber's tape. One job was close to finishing and the machine was about to stop. Employee #1 was in a hurry to finish the job. He had a metal cutter in his hand to cut the tape as the machine stopped. Accidentally his glove got caught in the sharp edges of the plumber's tape and it pulled his right hand and the metal cutter into the machine and resulted in the amputation of his right middle finger. Employee #1 was taken to Pomona Valley Hospital by the paramedics and was hospitalized for three days. Employee #1 stated that there was no guard on the machine to cover the pinch points and moving parts of the machine.
