Abstract: At 12:30 p.m. on July 17, 1998, Employee #1 was climbing a 30-foot extension ladder. While climbing, Employee #1 was carrying two small hangers over his right wrist. He was using both hands to hold rungs while climbing the ladder. As Employee #1 reached a level 14 feet above the ground, his right foot slipped off the ladder rung and he fell off the ladder. The fall resulted in multiple fractures of the foot and back.
