Abstract: Employee #1, a journeyman carpenter, Employee #2, an apprentice carpenter, and the job foreman were constructing a structural wood floor and roof framing system that was approximately 30 ft tall. The roof framing system was constructed in part of 30 ft purlins and beams. Employees #1 and #2 were to lay 4 ft by 8 ft wood panels in the 30 ft bay sections. Employee #1 directed his coworker to land a stack of panels in the middle of the bay on the roof. The roof panels laid in the bays on the roof structure were not adequately nailed. Employee #1 nailed the bottom panel of the stack being dropped through the subpurlin joist hanger and into the plywood sheet of wood frame panel. As the foreman was backing up the forklift, the forks dragged on the panel and possibly caught the purlin spreading the bay. All the panels of that bay fell through, and Employees #1 and #2 fell approximately 30 ft onto the concrete floor. Employee #1 was killed, and Employee #2 sustained injuries that required hospitalization. Both workers were wearing harnesses, but they were not tied off. Two catenary lines were observed on the eastern wall: the first was located toward the northern end of the eastern wall and was secured on both ends. The second catenary line was located at the southern end of the eastern wall. It was secured at one end to the parapet wall with a "bucket," the other end of was not secured. On the western wall, a third catenary line was secured at one end to the parapet wall with a "bucket"; the other end was neither tied nor secured. The catenary lines were identified by the employer as 7/16 in. Blue Water II static line rated at approximately 6,600 lb. Three 1 in. nylon webbing straps were wrapped around the purlins. The 1 in. webbing was identified by the employer as 1 in. tubular webbing with a rated capacity of approximately 4,000 lb.
