Abstract: At approximately 10:30 a.m. on November 26, 2003, Employee #1 was operating a drilling rig, an all-terrain-8 by 8 portable and limited access auger track drilling rig. He was to clean and reconstruct two wells, PZ11-35 and PZ11-11A. This particular auger drilling rig was engineered by the owner of the company Employee #1 worked for. This machine is comprised of a Japan Yan Mar, and was converted into an auger. The mast was fabricated by the owner of the company, and the rotary head came from a company called Griddings. The controls came from a local hydraulic house and there were no manufacture instruction. Employee #1 was strangled when his sweater was caught in the auger and killed.
