Abstract: On July 7, 2010 at 2:30 pm, a full-time, hourly, employee (EE1) of Worldfresh Express, Inc. sustained a serious injury. It was not reported to Cal-OSHA. It was discovered during a planned inspection on October 24, 2012. Worldfresh Express, Inc. is a warehouse that handles perishables. EE1 was working alone as a helper in the warehouse. His duties were to clean, cut foil for pallets, and clean the office and bathrooms. His immediate supervisor was EE2 who was not at the warehouse at the time of the accident. EE1 went outside to bring a large trash can (41" high by 71.5" long) into the warehouse. EE1 was pushing the trash can and his left pinky finger became caught between the edge of the trash can and a metal rack inside the warehouse. EE1 felt a pain in his left finger and saw his finger had been partially amputated. There were no witnesses. There was no violation of a Title 8 standard that was causal to the accident.
