Abstract: On April 28, 2011, Employee #1, a permanent employee, was working for his employer, at a facility where soy milk and tofu are manufactured. Employee #1 was attempting to unjam (unclog) waste soybean from the okara machine that removes soybean waste. His right middle, ring, and pinky fingers were amputated, most likely by the moving paddles of the okara machine. Employee #1 was taken to LA County-USC hospital and hospitalized for five days. The employer reported the accident to the Division on the same day it occurred. The serious injury was caused by Employee #1 failing to stop and deenergize or disengage the power source at the okara machine.
