Abstract: At approximately 10:00 a.m. On January 16, 2008, Employee #1 was cutting a notch on a door-jam using a Delta 10 inch Table (Uni-Saw Cat Number 34-806, Serial Number 88C1660). When the wood kicked-back, it pinched Employee #1's left middle finger between door-jam and table saw working surface. Employee #1's left middle finger was fractured and amputated. A coworker drove Employee #1 to West Hills Hospital, where the injury was bandaged. Employee #1 was sent home the same day. Employee #1 had surgery the next day. Employee #1 was employed by Garrett Woodworking, a high-end door and cabinet manufacturer. The employer did not report the accident to the Division of Occupational Health and Safety; the accident was discovered during a planned inspection by the High Hazard South Unit on May 2, 2011. The accident was caused by a missing antikickback device on a table saw.
