Abstract: At approximately 10:00 a.m. on February 25, 2010, Employee #1 was working at a meat and poultry processing plant. He was working alone in the Red Meat Department and was cleaning the Amfec Mixer/Blender (Model Number 510). While cleaning the mixer, he opened two hatch doors on the side of the machine and noticed a piece of meat that was stuck in one of the bolts used to attach the mixer blade to the body of the machine. The interlock for the mixer did not power off the mixer when the employee opened the hatches. He placed his left hand in the opening to manually remove the meat from the bolt, and his glove was caught in the moving mixer blade. Employee #1's left small and ring fingers were partially amputated but he was not hospitalized. The investigation revealed that the employee did not deenergize the mixer prior to cleaning it, and that the mixer did not have an operative interlocking device. It was also revealed that the employee had not been trained on hazardous energy control procedures or on the hazards related to performing cleaning operations for the mixer.
