Abstract: At approximately 8:30 on April 27, 2004, Employee #1, a machine operator in a plastic food container manufacturer, was operating the Forming machine, Identification Number 16, feeding in plastic sheets. While operating the machine, a piece of material became stuck in the chains. Employee #1 asked a coworker, a packer at the other end of the machine, to help her. While she was trying to remove the material with a knife, she instructed the coworker to turn the machine on. The coworker turned the machine on, and Employee #1's knife became caught in the chains, pulling her hand into the heated rails. Employee #1 sustained a burn to her left index finger, which was eventually amputated. She was not hospitalized for her injury. (This injury was not reported and was discovered during a planned inspection of the facility.)
