Abstract: At approximately 1:30 p.m. on January 26, 2005, Employee #1 was cutting wood pieces 2.125-in. wide by 32-in. long by operating from the control panel in front of the saw machine. The wood is first lined up for the measured cut. The saw blades are kept in a retractable compartment away from the operator's position. The 14-in. saw blade cuts several pieces at a time. There is a waste board (usually two product pieces and one waste board) on the top of the cut to protect the finished wood product. Once the operator activates the saw, a green light comes on and the blade comes out of its guarded compartment and a guard slides down over the cutting area. Employee # 1 states that the emergency bar was not working and that it was an older machine. The machine's normal operations have the bar guard lowering down slowly, yet Employee #1 claimed that on his last cut, the bar guard lowered down very fast onto his hand. The safety guards for this machine that should have prevented this accident did not enclosed the saw blades. Employee #1 was trained on the safety rules and procedures of the company as well as this saw machine. Employee #1 suffered the amputation of four fingers to his left hand and was hospitalized.
