Abstract: At approximately 10:00 a.m. on November 17, 2004, Employee #1, a machine operator, was working was working for his employer, a food products manufacturer. Employee #1 was working at a cup packing machine when he noticed that six sleeves had fallen into the chain and sprocket. Employee #1 opened the guard to remove the sleeves when the chain and sprocket amputated part of his index, middle and ring fingers. Employee #1 was taken to an industrial clinic, where he was transferred by paramedics to Western Medical Center in Tustin, California. At 4:59 p. m., the incident was reported to the California Department of Occupational Safety and Health.
