Abstract: On October 15, 2001, Employee #1, a truck driver for Desert Pumice Incorporated, amputated his right index finger while moving two 50-gallon drums of liquid chemical. Employee #1 was moving the two drums from a pick up truck bed to a pallet supported on the forks of a forklift. After the first drum was moved, the second drum was being rolled (on its bottom edge) to the pallet next to the first drum. Upon preparing to rest the drums, the other employees pushed the first drum. As the second drum was placed down on the pallet, Employee #1 lost his right index finger when his finger was caught between the top rings of the drums. Employee #1 was hospitalized to treat his amputation.
