Abstract: On February 7, 2001, Employee #1, a saw machine operator for K & Z Cabinet Company, Incorporated, lost several fingers and a thumb on his left hand while using an unguarded table saw. Employee #1 was using operating the Altendorf table saw within the countertop department. The Model Number was F-45, and the Serial Number was 86-5-39. Employee #1 was using his hands to hold down a piece of wood while directing an assistant employee to raise and lower the rotating saw blade with the use of a crank. The piece of wood held by Employee #1 "kicked back", causing Employee #1's left hand to make contact with the rotating blade on its descent. Employee #1 amputated his left thumb, index, middle and ring fingers, and lacerated his little finger. Plus, the table saw did not have a guard, spreader, or an anti-kick back device. Employee #1 went to a local hospital for treatment.
