Abstract: At approximately 3:55 p.m. on September 29, 2003, Employee #1, a house painter, was using a spray gun to paint the wall of a residence in Anchorage, AK. He was on a 25 ft extension ladder when the ladder kicked out and he fell between 15 and 16 ft onto the sloping driveway. Employee #1 suffered a fractured hip and was transported to Providence Hospital for treatment. Subsequent investigation revealed several contributing factors to the accident. Employee #1 had placed the ladder on a surface with a 10 to 15 degree downward slope. In addition, the asphalt driveway was slick because of intermittent rain showers. The angle of the ladder to the roof surface was not determined, but Employee #1 was believed to have overextended while spray painting from the ladder. Two rubber grommets normally placed on the end of the side rails of the ladder were found on the edge of the roof along with a slight indentation on the gutter lip from the ladder, suggesting the contact point of the ladder at the time of the fall. The ladder was not secured at either the upper or lower contact points.
