Abstract: On July 22.1077 Employee #1 was on a three-person crew hoe-ramming a section of street to prepare for digging it up. One coworker was on the trackhoe operating the ram while Employee #1 and the other coworker held plywood shields near the point of operation to keep debris from flying into the street and sidewalk. As they worked, the trackhoe's 8 ft tall hammer dislodged, fell over, and struck Employee #1 in the head. He was killed by the blow. A mechanical failure in the trackhoe's quick-coupler locking mechanism had caused the 8 ft tall hammer to dislodge.
