Abstract: Employee #1, a lumber feeder, had guided a piece of lumber into the feed rollers of a moulder machine when he noticed a metal staple imbedded in the board. From his work station, he activated controls to reverse the feed roller to move the piece of lumber to within his reach. For an unknown reason, the feed rollers failed to move the piece of lumber. The employee then left his work station and entered the machine enclosure while the machine was still running. As he pulled the staple out, his right hand slipped and contacted the high-speed cutting head. His thumb and first three fingers were pulled into the cutters and were amputated. He was hospitalized with his injury.
