Abstract: On November 21, 2001, Employee #1, a drywall taper, placed a ladder on top of a scaffold platform erected two stages high and placed the top of the ladder against the interior wall of a skylight opening on a residential structure under construction. However, only one of the wheels on the scaffold was locked. When Employee #1 climbed the ladder, the sideways thrust exerted by the ladder caused the scaffold to pivot. Employee #1 fell approximately 17 feet to the floor, fracturing his wrist and pelvis. Employee #1 was hospitalized for treatment on his wrist and pelvis.
