Abstract: At approximately 11:00 a.m. on September 12, 2001, Employee #1 was unloading a 54-inch-diameter steel pipe from a flat bed trailer onto the forks of a front end loader with a forklift. The operator of the loader picked up the pipe from the trailer tilted back using the forks, while Employee #1 placed a piece of lumber from the trailer on the ground. Employee #1 reached for the lumber at the same time the operator lowered the forks. While the forks rested on the ground, the steel pipe, which weighed approximately 5,000 pounds, rolled off the forks and crushed Employee #1 between the flatbed trailer and the pipe. Employee #1 was transported to Emanuel Hospital in Portland, Oregon, where he died during surgery.
