Abstract: At 11:30 a.m. on July 17, 1997, Employee #1, of JB Enterprises, Inc., was painting from a 24 ft extension ladder. The rubber feet of the ladder slipped out from underneath him when he was at a height of approximately 10 ft. Employee #1 fell onto a wooden deck surface, fracturing his left ankle.
