Abstract: At 9:30 a.m. on February 23, 1999, Employee #1 sustained lacerations to his head after falling into a confined space area. He was working on an asphalt storage silo bolting wear plates to the conveyor chute, which feeds material into the silo. As he was bolting the plates, he fell approximately 30 feet into the silo. As a result, he sustained lacerations to his head. He was hospitalized for treatments for his lacerations.
