Abstract: At approximately 3:30 p.m. on April 23, 2003, Employee #1 and a coworker were on a 30-ft-high pier mock-up table on a bridge construction site receiving and unloading the hoisted materials. A crane operator had just hoisted to the table a bundle of PVC pipes that was rigged in a two-way bridle arrangement, with a 0.375-in., 7-ft-long tag line attached to one of the slings. After Employee #1 and the coworker, who were positioned on each side of the bundle, released the chokers, Employee #1 radioed the crane operator to raise the sling. As the sling was being raised, the tag line became snagged and snapped. The upper section of the tag line recoiled and wrapped around Employee #1's right index finger He was hospitalized for a dislocation and abrasion.
