Abstract: At approximately 5:40 p.m. on July 16, 2001, Employee #1, a molding machine operator, got his right arm caught between the right pattern carrier assembly and the right pattern clamp of a SHALCO Shell Molding Machine, Model Number DSM-3 Twin. Employee #1 tried to grab an ejector pin which was hooked to the lifter when the pin fell into the clamp's area of operation. Employee #1 was transported to the Alta Bates Medical Center, where he was treated for second-degree burns and underwent skin grafting.
