Abstract: At approximately 2:00 p.m. on November 5, 1999, Employees #1 and #2, both drywall installers, fell from an elevated, 8-ft-high work platform when it tipped over. One of the wheels of the scissor lift had dropped into a 2-ft by 2-ft by 6-in. deep cut out in the concrete floor as they were moving the platform. Employee #1 was hospitalized with a broken right radius and ulna. Employee #2 was hospitalized with a broken right elbow and fractured pelvis.
