Abstract: Employee #1 was on the elevated platform of a rough terrain forklift truck along with HVAC equipment that he was starting to off-load. As the equipment was moved, the platform became unbalanced and he was thrown off it. Employee #1 fell 24 ft to the ground, and was killed.
