Abstract: On December 6, 2001, Employee #1 fell 15 to 20 feet while working from an adjustable tower scaffold. At the time of the fall, Employee #1 was holding a 2-inch hose. One end of the hose was connected to a mechanical pump. The other end, the nozzle, was held by another employee. Concrete was sent through the hose and was being poured into cinder blocks. No employees at the site saw Employee #1 fall. Employee #1 received fatal injuries in the fall.
